Applications to St. Mary School are accepted throughout the school year. Please note that all 4 year old

kindergarten students must show a copy of the birth certificate to enter school.

Transfer students are asked to provide a copy of the school records from their former school.

The State of Wisconsin requires the following boosters for kindergarten entrance:
DTaP#5
Polio#4
MMR#2
Chickenpox/Varicella#2

Please send the immunization dates to school after receiving the booster shots.

There are many changes with the coverage of vaccines through public health. Coming this October, many people will no longer be able to receive vaccinations at school (or health department, or any other public health organization) if they have health insurance (other than MA). People will be required to use their Dr. for vaccines.
